糖尿病风险评估系统对糖尿病早期筛查的研究分析

尽管人们对糖尿病早期诊断的重要性已经熟知,但在人群中进行大规模的筛查仍然存在困难。医院及社区最常用的仍然是传统的糖尿病筛查方法,即空腹血糖、糖化血红蛋白(HbA1c)检测和葡萄糖耐量试验(OGTT)这三种检测方法。但这三种方式均为有创性的检测,并且在部分人群中的敏感性较低。除此之外,空腹血糖检测会使得绝大多数餐后血糖高的糖尿病患者出现漏诊。目前空腹血糖检测主要采用静脉或毛细血管取样的方法,但灵敏度不够。OGTT虽然准确率高,但操作比较复杂,需要多次抽血,检测周期长。HbA1c的测定成本高,灵敏度低,且没有完全标准化的分析方法。事实上,如果仅用空腹血糖诊断糖尿病,灵敏度可低至40%~([1,3])。糖尿病风险评估系统是无创性的,采用多电压直流刺激法(Multiple Potential Direct Current Simulation)对受检者的汗腺施加多组电压,通过欧姆定律测量汗腺内不同离子在临界分解电压附近的电化学反应,准确的评估汗腺功能,从而检测末梢神经病变情况及评估β细胞的功能状况。
